FAQ: Which affiliate signup fields are available?

Simple Affiliate collects your affiliates' names and email addresses, and lets you add social profiles, contact information, custom questions, and consent checkboxes. This FAQ lists the available fields and the type of information each one accepts.

Manage the extra fields in Settings > Signup > Affiliate signup options. The fields an applicant sees depend on your settings and whether they register through the storefront form or their Shopify customer account.


Available signup fields

The input types below describe the information collected. A field's appearance can differ between signup forms. For example, a profile URL is a link, while a follower count must be a whole number.

First name Text The affiliate's first name.
Last name Text The affiliate's last name.
Email Email address The affiliate's email address. Applicants enter it on the storefront form. Signed-in customer-account signup uses their Shopify account email.
Social media name Text A social media name or nickname, separate from the TikTok and Instagram profile links.
TikTok profile URL URL A full TikTok profile link.
TikTok username Text, automatically filled Extracted from the TikTok profile URL. The applicant does not edit this field directly.
TikTok followers Numeric, whole number An optional follower count of zero or more.
Instagram profile URL URL A full Instagram profile link.
Instagram username Text, automatically filled Extracted from the Instagram profile URL. The applicant does not edit this field directly.
Instagram followers Numeric, whole number An optional follower count of zero or more.
Phone number Telephone number (text on the classic form) A phone number, with a country-code selector on the storefront form.
Birth year Dropdown, numeric year A year selected from a list, not a full date of birth.
Custom 1 Text An answer to the first question or label you define.
Custom 2 Text An answer to the second question or label you define.
Custom 3 Text An answer to the third question or label you define.
Email marketing consent Checkbox Confirmation that the applicant agrees to receive email marketing.
Affiliate agreement Checkbox Confirmation that the applicant accepts your affiliate agreement. You can supply a link to the agreement.

Enabling TikTok profile or Instagram profile shows that platform's URL, automatically filled username, and optional follower count. Setting a profile to Required requires its URL, not its follower count. These profile fields appear in the storefront and new customer-account forms, not the classic customer-account form.


Fields that depend on customer accounts or payouts

These fields are not shown on every signup form. Payment fields depend on the payout method and payment-detail collection settings.

Password Password For a visitor creating a classic Shopify customer account through the classic affiliate signup form.
Repeat password Password Alongside Password, to confirm the same password. Neither password field appears in the storefront form or new customer accounts.
Payment email Email address When manual payment details are collected as an email address.
Payment details Text When manual payment details are collected as free text.
SWIFT / BIC Code Text When this field is enabled for collecting manual bank payment details.
Routing / Bank Code Text When this field is enabled for collecting manual bank payment details.
Account Number / IBAN Text When this field is enabled for collecting manual bank payment details. It is text, even when an account number contains only digits.
PayPal email Email address An optional field when PayPal is enabled and is the payout method used for the signup.

Important Rules to Remember

  • Visibility: Social media name, TikTok profile, Instagram profile, Phone number, Birth year, and each custom field can be Hidden, Optional, or Required. Hidden fields do not appear; optional fields can be left blank; required fields must be completed. The classic customer-account form does not block signup when a required Birth year is left blank; the storefront and new customer-account forms do.
  • Custom questions: In settings, these are named Custom Field 1, Custom Field 2, and Custom Field 3. You can change their labels, but all three remain text inputs. A label asking for a number or a yes/no answer does not turn the field into a numeric input, dropdown, or checkbox.
  • Consent and agreement: These use on/off switches in settings. When enabled, their checkboxes must be checked to complete signup.
  • Custom referral link: This is a setting, not an applicant input field. When available and enabled, Simple Affiliate automatically creates a custom referral link when the affiliate registers.
  • Approval: Manual approval, Automatic approval, and Invite only control how affiliates join your program. They are not signup fields.
