How to Set Up a Custom Affiliate Signup Form

Custom affiliate signup forms let prospective affiliates register on any page of your Shopify store without logging into a Shopify customer account first. This guide covers creating the page, adding the Affiliate Signup app block in the theme editor, and turning the form on in Simple Affiliate.

Availability: Storefront signup requires Shopify New customer accounts. Stores using classic customer accounts can add the app block, but form submissions are rejected. After email verification, applicants authenticate with a one-time passcode on the store login page.

How the registration flow works

Applicants move through these steps:

  1. Form submission: The applicant fills out and submits the registration form on your store page.
  2. Email verification: Simple Affiliate sends an automated email with a verification link.
  3. One-time passcode: The verification link opens your store's login page. The applicant authenticates with a single-use passcode.
  4. Dashboard access: After authentication, the affiliate can open their affiliate dashboard.

1. Create a page in your Shopify admin

  1. In your Shopify Admin, go to Online Store > Pages.
  2. Click Add page.
  3. Enter a Title for the page, such as Affiliate Signup.
  4. Set visibility to Visible.
  5. Select the page Template (for example, Default page).
  6. Scroll to Search engine listing and copy the URL handle (for example, pages/affiliate-signup). You will enter this path in Simple Affiliate later.
  7. Click Save.

2. Open the page in your theme editor

  1. Go to Online Store > Themes and click Customize.
  2. Click the page selector dropdown at the top center of the editor.
  3. Click Pages.
  4. Search for the page title from step 1 (for example, Affiliate Signup) and select it.

3. Add the Affiliate Signup app block

  1. In the left sidebar under Template, click Add section or Add block.
  2. Open the Apps tab.
  3. Select Affiliate Signup.

4. Configure the block settings

  1. Click Affiliate Signup in the left sidebar to open its settings.
  2. Set Form columns to Single column or Two columns.
  3. Adjust Padding and Width. Padding of 0 keeps your theme's default section spacing.
  4. Optionally set color overrides for background, text, input border, input background, and the error banner. Leave color fields blank to inherit your theme.
  5. Click Save in the top-right corner of the theme editor.

5. Enable the custom form in Simple Affiliate

  1. Open Simple Affiliate from your Shopify apps menu.
  2. Go to Settings > Signup.
  3. Find the Storefront signup form card.
  4. Turn on Use a custom signup form.
  5. Enter the page path from step 1, including the leading slash (for example, /pages/affiliate-signup).
  6. Click Save.

Important Rules to Remember

  • New customer accounts: The form only accepts submissions on stores using Shopify New customer accounts. Classic customer accounts receive an unavailable response.
  • Manual approvals: If manual approval is enabled, pending-affiliate notifications go to your admin only after the applicant verifies their email and authenticates with the one-time passcode.
  • Dashboard URL: Turning on a custom signup form hides the default registration form on your affiliate dashboard URL. That URL becomes a login page. Unauthenticated visitors see a prompt that sends them to your custom signup page.
  • Customer accounts: Applicants can submit the form without creating or logging into a Shopify customer account first. Authentication happens after they click the verification email.
  • Fields and copy: Field order, required fields, labels, and agreement wording are set in Settings > Signup, not in the theme editor.

What happens next?

Visitors can submit the form on the page you created. They receive a verification email, authenticate with a one-time passcode, then reach the affiliate dashboard. To restyle the form after it is live, follow How to style your custom affiliate signup form.
